Parent Release Form
All teens will be required to have a medical release signed by their parents when attending RedLine events (besides normal services). That form is available for download below.
If parents won’t be coming with their teen then this form should be filled out ahead of time & the teen bring it with them. If parents are dropping teens off, it can be completed at the door.
